I received an email from the Union that I will be receiving this form so I can fill out my tax forms. It is proof that I had insurance.
Does anyone know anything about this form?

I didn't have Kroger insurance but I do have BCBS. I don't remember any form for taxes.
Nevermind. It appears I only need it if I was not covered by Kroger and got insurance on my own.
Just did a rough draft of my taxes. It explained it in the program.
Well, I did get a 1095-C in the mail today. It just shows that I was covered by the employer health plan for the entire 12 months.
I guess anyone that isn't covered for the entire year will be getting a 1095-B.
In our contract, if the part timers fall below a certain average number of hours a quarter, they lose insurance until they make that average again.
