I'm a former employee, and I didn't know my EUID and the process was horrible! Hopefully this will help some of you:

Call: 18009528889 (ext 47)
They will get you the last four digits of your EUID

Go to: www.mytaxform.com

Username: social security number

pin: last four digits of your EUID + your birth year (all four digits)

The man I spoke to said the website won't work on macs/apple computers, so make sure you're on a PC. Good luck!

I'm a former employee and this is a nightmare! I have done everything to access my W-2 an i STILL cant get in and the corp or my former place of work wont help me. All I got in the mail was a 1095-C.

Still havent got my tax form from Kroger I am a former employee is anybody know a number to call or what I need to do to get my tax form. The store does not really want to help me.

Go to www.mytaxform.com - then enter their company code: 10575

It will ask you to make an account with your personal info and social security number. This is safe and secure. After you have created an account you can download your W-2 directly from the website, or have it mailed to you.
