Theoretically, your immediate supervisor or your store management. Worst case, you could talk to a lawyer or try for a restraining order. I know there is a number you can call, but I will check and post later.

Yes.  I filed quite a few.  You can file a complaint anomalously or with name.  I filed one recently against a customer that keeps coming in to try and get a coworker of mine unjustly fired over garbage the customer started.  List events with dates and/or time stamps if possible. If you can get coworker to sign for witness signature for each event.  Google a few harassment letters for examples.  Legal is generally good to keep wordiness down.

 Your co worker sounds like a lazy. They should have exceeded the customers needs. Nothing will happen to the customer.
