I am planning on putting my two weeks in this Saturday. I am going to write a letter of resignation, I was wondering who do I address/give this letter to(my department manager one, co-manager, or the actual store manager)?

Just write "XXth of Xxxxxxxx will be my last day to be working here. Thank you for all the opportunity working here has given me" and give it to your store manager.

My store has forms that they give people who quit where you write your name, your EUID, your last day, why you are quitting, and a spot for a note. They keep them in a folder at the service desk.
As long as you communicate that you are leaving, it shouldn't matter. What is important is your name and your last day, they should be able to figure it out from there. I'd say to give it to your department head as they are the one who schedules you, but a manager or your HR person should be fine also. Your department head might not see it for a while (it will probably get lost on their desk), but you still would be giving 2 weeks and you would be able to get hired again.

Do not give it to your department head. Just tell them or make a copy for the department head. Give the note directly to your Store Manager. If you give it to the Department head, only, your Store Manager can say that they never received your notice and could cause all sorts of trouble, down the line, with Kroger saying that you were for for "No call; no show". Thus giving your profile "do not rehire" status.
