ok, so if i work at kroger, in the pharmacy, and i would like to leave a disapproving comment about my co-workers or managers, would they be able to tell who i was by looking me up in the system (i'm not asking if they could logically figure out who i was)?

if the answer to that is yes, and my relative has a kroger card there but is a regular shopper, could i fly under the radar doing a survey under their card?

While you are right, you don't have to be such a dick about it.

Is it possible to word the comment so it sound like it's coming from the point of view of a customer?  Something like, "The other day I wasn't at the pharmacy counter and I saw..."

Why not just make up an email to put where it says email and make up a card number to leave the survey?

Name: (make one up)
Card number: (make one up)
E-mail: (just type one up no need to make a new email)
Phone number: (type in any random number)

Ther you go. No one will know its you. When i do "surveys" i type in any random email adress in the email part and any random phone number. They will NEVER find out that its me. And yes, when we have "incentives" those surveys i did helped get us those incentives
